If you are a parent applying on behalf of a student

• If your child has applied to GMCMF before, please enter their first and last name and the same email address they used last time to apply to find their account.

• Applicant's name and UNIQUE email address must be used on this form. DO NOT use parent names or emails on this form.
• If you are a parent applying on behalf of your child, please be sure you have access to their email inbox. Using a parent's email address with a student's name will cause tangles in our database.
